New Patients
We are delighted to welcome you to our practice and are pleased that you chose us to serve your dental needs! Just download the New Patient Information Form and the HIPAA Forms and get started! When you’re finished you can bring them along with you or email them to us.
Insurance
Prior to your first visit, we will contact your insurance company to determine which services your individual policy covers and at what percentages.
Even though we participate with many insurance companies such as United Concordia, Delta, MetLife, Guardian, UPMC, United Health Care, Cigna, and Aetna, your coverage will depend upon your individual policy and may not cover your treatment in full. We realize that dental insurance can be confusing. We will communicate to you what your insurance estimate will be prior to treatment. Understand that insurance estimates are not a guarantee of payment by your insurance provider, they are only an estimate. Coverage on policies from the same company can range from no coverage for certain procedures, to complete coverage on others.
Payment
We accept cash, checks, debit cards and credit cards. We also provide the opportunity to finance larger cases through CareCredit. Once your dental plan has been verified and your insurance coverage determined, arrangements for payment of non-covered services can be made in advance of your treatment.
Yes! Whether you use your healthcare credit card to cover your deductible, or to pay for treatments and procedures not covered by insurance*, CareCredit helps make the health, wellness and beauty treatments and procedures you want possible today.
*Subject to credit approval.
We expect payment at the time of service. If you are concerned about your dental insurance or have questions pertaining to it, please do not hesitate to ask for assistance. We will be happy to help. Call us at 412-338-0678 or visit our Contact Us page.
